Payment Terms of Service

These Payment Terms of Service ("Payment Terms") govern billing for compounding and pharmacy services ordered by clinics and providers through the American Wellness Pharmacy Provider Portal (the "Portal"). By authorizing a payment card and submitting an order, you agree to these Payment Terms.

1. Authorization to charge

When you place a card on file and authorize billing, you authorize American Wellness Pharmacy ("AWP") to charge that card for all compounding and pharmacy services ordered through the Portal by your clinic and its authorized users. You represent that you are authorized to permit charges on the card provided.

2. Billing descriptor

Charges will appear on your statement as "American Wellness Pharmacy."

3. When you are charged

Unless otherwise agreed in writing, charges are applied when an order is processed. Pricing reflects your clinic's assigned pricing tier and any applicable discounts in effect at the time the order is processed.

4. Declined or expired cards

If a charge is declined or a card has expired, processing of affected orders may be paused until valid payment information is provided. You are responsible for keeping your billing information current. To update billing or card information, contact our accounts team at accounts@americanwellnesspharmacy.com or (702) 405-9500.

5. Compounded medications — returns

Because compounded preparations are made specifically for an individual patient, they generally cannot be returned, exchanged, or refunded once dispensed, except where required by law or in the case of a verified pharmacy error. Questions about a specific order should be directed to the accounts team.

6. Billing disputes

If you believe a charge is incorrect, please contact us within 30 days of the charge so we can review it. We will work in good faith to resolve billing questions promptly.

7. Changes to these Payment Terms

AWP may update these Payment Terms from time to time. Continued use of the Portal after an update constitutes acceptance of the revised Payment Terms.
